Gây mê hồi sức trong phẫu thuật tách trẻ sơ sinh dính nhau

Phẫu thuật tách trẻ song sinh dính nhau là một công việc hiếm gặp, phức tạp và chưa từng thực hiện ở Bệnh Viện Nhi Đồng 2. Đây là một siêu phẫu cần nhiều chuyên khoa tham gia: phẫu thuật, gây mê, hồi sức, điều dưỡng... Để gây mê cho loại bệnh nhi này, chúng tôi lập và tập huấn một đội ngũ gây mê hồi sức hùng hậu; tham khảo thông tin trên sách báo, mạng Internet và các đồng nghiệp có kinh nghiệm; kế đến, chúng tôi lên kế hoạch gây mê, tiên liệu những tình huống có thể xảy ra. Trang thiết bị, dụng cụ gây mê được tính gấp đôi trong một phòng mổ rộng nhất.

Kết quả: Ngày 02-11-2005, phẫu thuật hai bé gái song sinh, 17 tháng tuổi, cân nặng chung 23kg, dính nhau ở ngực và bụng. Thách thức về phía gây mê phải đạt được: nhận dạng các cơ quan bị dính, thông khí, đường truyền, mất nhiều máu, thân nhiệt ... Hai bé dính 1/3 xương ức, gan, cơ hoành, thông khoang màng tim. Phẫu thuật hoàn thành sau 8 giờ, gây mê 10 giờ, truyền 500 ml máu cho cả hai.

Kết luận: Thành công trong phẫu thuật tách trẻ song sinh dính nhau là nhờ vào các thông tin khoa học, sự hợp tác của đa chuyên khoa, là cơ hội nâng cao kiến thức và thực hành.

 

SUMMARY

ANESTHESIA IN SEPARATION OF CONJOINED TWINS

Objective: Prior to anesthesia management for conjoined twins, an anesthesia team was assembled. The first task of our anesthesia team was to gather information in the scientific litterature, on the Internet and consult others experienced colleagues. Then we discussed the anesthetic plan, anticipate problems and establish contigencies. The anesthesia equipements were duplicated in our largest operating room.

Method: Surgical seperation of conjoined twins is a rare, complex procedure. It had never been performed at Hospital Nhi Đồng II. It required a participation of co-ordinated multidisplines: surgery, anesthesia, intensive care and nursing.

Results: In Nov 2nd 2005, surgical seperation of two girls twins, 17 months, total weight 23 kg, joined at the thorax and abdomen, was performed. The challenges encountered in anesthesia for these twins included identifying anatomical conjunctions, airway management, acquiring vascular access, the potential for enormous blood loss and maintaining normothermia. After 8 hours surgical seperation was finished. They were joined at 1/3 inferior sternum, liver, diaphragm and their pericardiac cavity was communicated. The anesthesia duration was 10 hours, and requiring transfusion was 500 ml for both.

Conclusion: Successful management of conjoined twins relies on close communication and cooperation of all members of multidisciplinary team. This is a great opportunity to educate our physicians in training.
